Exercise-Induced Pulmonary Hemorrhage, or HPIE, is a condition that can occur in all horses during intense exercise. Many believe that horses with HPIE will bleed from the nose, but studies show that only 10% of horses have blood in their nostrils.
HPIEoccurs when small, deep blood vessels rupture in the lungs, causing red blood cells to invade the airways during intense exercise.
This happens because as the horse intensifies its exercise, it needs more oxygen to energize its heart and muscles; to do this, the horse breathes more heavily and faster. When galloping fast, a horse moves about six tubs of air in and out of its lungs every minute.
When running, imagine the horse as a large blower, inhaling and exhaling air about 120 to 140 times per minute. At the same time, the heart rate and the heart increase to pump more blood and supply more oxygen to the muscles during this work. At that moment, a very fragile membrane called the pulmonary capillary membrane, which separates the oxygenated blood from the air sacs, experiences very high pressure on one side, while on the other, there is a suction force pulling the fragile membrane as the horse inhales.
Combined, these forces cause the rupture of this membrane and the escape of these red blood cells into the airways, resulting in EIPH (Exercise-Induced Pulmonary Hemorrhage). This syndrome is classified from grade 0 to grade 5. See below how each grade presents:
Grade 0: Absence of visible blood in the trachea;
Grade 1: Presence of traces of blood (thin streaks) in the trachea;
Grade 2: Presence of blood streaks less than 5mm in the trachea;
Grade 3: Presence of blood streaks larger than 5mm and smaller than 15mm in width, or multiple streaks, occupying up to one-third of the trachea.
Grade 4: Presence of abundant blood streaks larger than 15mm in width and accumulation of blood in the trachea (more than one-third);
Grade 5: Nasal hemorrhage (epistaxis) and/or abundant blood accumulated from the trachea to the oropharynx.
